If you are interested in participating in the 2011 SWOTT, please complete the following SWOTT Entry Form and submit the form by pushing the SUBMIT BUTTON, or you can print and mail it.  Please mail your check so that it is received by Thursday, September 30th to:

To pay by Credit Card or PayPal:  AFTER you have filled out the form and clicked "SUBMIT", you will be returned to this page. The form will have been submitted and you do not have to fill it out again, but just scroll down to the PAY NOW button and click it.  You will be taken to the PayPal site, where you can pay by your credit card or PayPal funds.

The entry fee for the tournament this year has been lowered to $195.00 This includes everything -- balls, prizes and all scheduled events including meals and refreshments for the player and his spouse or date. However, players who play mixed doubles with someone other than their spouse/date, who is not covered by another entry fee or sponsorship, must pay an additional $75.00.

  A letter of confirmation containing hotel information and an update on the social events will be sent when your Entry Form and appropriate Fee are received.
